However, to initiate your order, your representative will need the information below. Please have these items at hand when you call:
- Your full legal name.
- Your physical address, including apartment or suite number.
- The name of your residence’s previous tenant, name of the closest neighbor with phone service, and landlord’s name and number.
- Type of service you want:
- Business
- Residential
- Internet
- How you want your name to appear in the directory and with directory assistance.
- Your choice of a long-distance provider.
- Information about your previous telephone service, including telephone number, city and state.
- Employment and credit information.
- Business ownership information if applicable.
